Valley Regional iscommitted to providing the best health care for the residents in the Rio GrandeValley.--Valley Regional MedicalCenter--is a 214 Licensed Bed Facility with over 200physicians representing over 25 specialties; Valley Regional Medical Center isa leader in a vast array of high quality inpatient and outpatient programs andservices. --What youwill do in this role:----Provides casemanagement services for both inpatient and observation patients as assigned----Identifies patients whoare at risk for adverse outcomes during the transition from one level ofcare/setting to another----Performs acomprehensive assessment of psychosocial, medical and discharge needs ofpatients/family along with an assessment of resources appropriate and availableto the patient/family----Reassesses thepatient's clinical condition as indicated. Considers patient's readmissionstatus or risk of readmission and develops strategies to mitigate includingeducation on appropriately accessing healthcare resources, preventativeeducation, and community based resources----Coordinates the plan ofcare and drives the discharge plan by collaborating with the multidisciplinaryhealth care team and in particular with the patient's physician to facilitate asuccessful care transition----In partnership withSocial Services, the RN CM is responsible for ensuring the post-acute medicalneeds and level of care are appropriate----The RN CM isresponsible for timely referral to Social Services when risk factors forpsychosocial determinants of health are identified----Involves patient,family/responsible/significant others in identifying and clarifying needs andexpectations to develop mutual and realistic goals----Evaluates progressionof care using evidence-based tools and approved criteria (InterQual) throughoutthe episode of care; escalates progression and transition of care issuesthrough the established chain of command----Makes appropriatereferrals to third party payer, disease and case management programs forrecurring patients and patients with chronic disease states----Facilitates patientthroughput with an ongoing focus on an effective care transition, quality andefficiency----Documents professionalrecommendations, discharge plan, care coordination interventions, and casemanagement activities to effectively communicate to all members of the healthcare team------Align patient's needswith available resources to ensure a safe discharge / transition----Acts as a liaisonthrough effective and professional communications between and with physicians,patient / family, hospital staff, and outside agencies----Actively seeks ways tocontrol costs without compromising patient safety, quality of care or theservices delivered----Directs activities toidentify and provide for the needs of the under-resourced patient population toinclude patient education activities, patient assistance programs, andcommunity-based resources----Participates inperformance improvement activities including, but not limited to, identifying,documenting and intervening when avoidable days occur----Adheres to establishedpolicy and procedure and standards of care; escalates issues through theestablished chain of command timely----Demonstrates knowledgeof regulatory requirements, HCA Ethics and Compliance policies, and qualityinitiativesServe as an advocatefor patient's rights, needs, and values; ensuring that patients' ethnic,cultural, or religious values, beliefs, preferences and needs are consideredand aligned----Qualifications Qualifications RN with current state licensure; BSN preferred Three years' clinical, hospital nursing experience required with preference given to those with 2 years of case management experience or 2 years of critical care experience Certification in case management or utilization review preferred InterQual experience preferredSkills------------------Ability to establishand maintain collaborative and effective working relationships------------------Ability to communicateeffectively in oral, written and electronic formats------------------Demonstrates analyticaland critical thinking abilities with proactive decision-making and negotiationskillsHCA Healthcareranks on Fortune's list of Most Admired Companies for three consecutive yearsand HCA ranks 63rd on the fortune 500 list.
