Valley Regional Medical Center is a 214 Licensed Bed Facility with over 200 physicians representing over 25 specialties; Valley Regional Medical Center is a leader in a vast array of high quality inpatient and outpatient programs and services, including: 24 Hour Level III Trauma Emergency Department Cardiology Open Heart Surgery Neurosurgery Endoscopy Imaging Women's Services Level III Neonatal Intensive Care Unit Valley Regional Medical Cente r is currently looking for a Nurse Navigator Care Assure & Advanced Clinicals Specialist (RN) - HiTech Responsibilities Nurse Navigator: The Care Assure Nurse Navigator is responsible for the programs and resources that support the navigation of patients through complex healthcare environments for the optimization of care and outcomes. S/he facilitates patient follow-up through coaching, information transfer, and communication in order to improve continuity of care, enhance the medical management of complex disease, and streamline the intervention process. The incumbent guides patients through the health care system by assisting with access issues, developing relationships with service providers, and tracking interventions and outcomes. Serves as a point of contact for referring physicians, patients and caregivers to provide resources and assistance with accessing clinical and supportive care services. Primary Responsibilities: -- Reviews patient case lists generated by technology-enabled data triggers to identify patients that could benefit from intervention, while tracking outcomes of referrals and treatments and maintaining detailed records that measure effectiveness of the program. -- Reviews clinical progress of patients and coordinates communications to referring, consulting, and primary care physicians, notifying an MD if there is a clinically abnormal finding. -- Evaluates and provides input regarding the utilization of resources to meet the needs of the target population. -- Plan and delivers weekly status updates. -- Maintains and role models a leadership/customer service role in interacting with colleagues, physicians, guests, patients, and vendors. Responsibilities Advanced Clinicals Specilaist: Under the direction of the Director of Advanced Clinicals, the Advanced Clinicals Specialist is responsible for ensuring ongoing training, support and refinement of the Advanced Clinicals and Meaningful Use initiative as well as additional support for clinical applications throughout the facility. This individual participates in the company-wide community for CPOE changes and works with their facility peers to ensure advanced clinical applications are well supported throughout the facility. Primary responsibilities include the following: -- Duties & Responsibilities o Assist in the implementation and support of division-wide standardization, utilization, integration, and optimization activities related to CPOE and other clinical applications and tools. o Assists in trainings and/or remediation efforts for physicians and other providers in the PatientKeeper application o Works with the EHR Analyst to support nursing staff documentation in the Meditech system o Assists in the implementation of any new projects assigned to the Advanced Clinicals Department o Supports education plans focused on the CPOE clinical initiative, Meaningful Use and additional clinical applications as developed and implemented o Resolution of complex issues and requests escalated from the division Service Desk or other division or facility service resources. o Ongoing monitoring of various advanced clinicals usage trend reports. -- Data Quality Management o Manages unsigned orders in PatientKeeper concurrently and post discharge o Manages incorrectly signed and rejected orders in PatientKeeper concurrently and post discharge o Manages unsigned Notewriter notes concurrently o Collaborates with physicians to correct erroneous Notewriter entries or supervision physicians assignments in PatientKeeper o Collaborates with HIM to close out charges that have been designated for administrative closure Qualifications Position Requirements: Education: Bachelor's Degree in Nursing highly preferred. Masters degree preferred Experience: Minimum 3 - 5 years relevant specialty area experience (i.e. Cardiovascular, Orthopedic, Neuro, Spine, Oncology) specialty area experience strongly preferred. Knowledge and application of pertinent computer programs and imaging software (I.E. MUSE, PACS, etc. Minimum Training Advanced Clinicals Specialist: -- Proficient in word processing and computer systems -- Knowledgeable of Meditech charting and reporting systems -- Preferred knowledge of HIM regulations and processes -- Understanding of Medical Staff Bylaw Rules and Regulations for Medical Records -- High level communication skills with a variety of roles both verbally and written Minimum Work Experience: -- Previous experience with Electronical Health Systems -- Hospital experience required -- Three to five years of experience in clinical applications support or equivalent preferred License/Registration/Certification: -- Current RN License -- BLS
