POSITION SUMMARY: The Patient Navigator will assist clinical team members by performing a variety of duties for patients within the patient focused care system and serves as a single point of contact for referring physicians, patients, and caregivers to provide resources and assistance with scheduling additional imaging procedures ordered for continuation of care within DHR Health. Facilitates patient appointments for breast imaging procedures. POSITION EDUCATION/ QUALIFICATIONS: Graduate of accredited Associates or Bachelor's Degree program in nursing, radiology, or health-related program required. Current license in the State of Texas preferred.Current BLS certification required.Breast Navigator Certification within 24 months required, if navigating Breast Imaging.CT Lung Screening Certification within 24 months required, if navigating the CT Low Dose Lung Cancer Screening Program. Must be able to be sensitive to cultural and bilingual issues.Good written and verbal communication skills required. Ability to read, write and speak English required, Spanish preferred.Ability to communicate clearly and concisely with all levels of nursing, administration, and physicians. JOB KNOWLEDGE/EXPERIENCE: Work experience and knowledge in corresponding imaging (i.e. breast imaging, CT) preferred.Must have excellent customer service skills.High degree of competency/experience in general. Requires good communication and organizational skills. Requires reasoning ability and strong decision making skills.Requires excellent leadership and interpersonal skills.Excellent presentation skills, preferred.Ability to implement professional and community based education programs.Microsoft Office competency required. POSITION RESPONSIBILITES: Promotes the facility mission, vision and values by effectively communicating them to others.Considers mission, vision and values in developing services, standards and practices.Assist patients in scheduling their additional imaging based on the radiologist's recommendations and physician's orders.Serve as an essential link between patients and all other care providers and assist to facilitate appointments for consults and support services as needed.Develop patient education information and tools.Follow patients throughout the imaging care continuum.Work with staff to collect data, track outcomes, and support strategic planning processes.Deliver quarterly report to dedicated committees which demonstrate outcomes and performance improvement activities.Adhere to established productivity benchmarks.Responsible for outreach efforts to establish and maintain positive working relationships with key customers (physicians, office staff, diagnostic staff, nurses, radiology staff, social services staff, and radiation oncology staff, etc.).Attend assigned trainings and meetings.Other duties as assigned.
